I have just finished filling in a form which asked all sorts of questions about my medical history.

The next page has this -

If you have answered yes to any of the questions 1-18 above please give further details below. Please use the back of the page should you need more space. Please state the nature of the problem, when it occurred, length of time it lasted, the effect it had on you, the name of any medication or treatment and if it still affects you in any way etc.


All my teeth were removed when I was 4 years old - I can't remember too much about it apart from the beds that revolved if you moved (you were strapped in) and left you hanging up-side down.
Oh and some other kid broke the head off of my Donald Duck toy car

Hate those forms ~ mostly created by idiots with time on their hands.

I had one once that asked "Have you ever been admitted to hospital or had an operation ~ if yes give FULL details"
So I told them all about having my tonsils out when I was 4.

I would have much more to tell them if they asked now.
